DGIndex showed 16:9 A/R. I set Gknot to NTSC 16:9, auto cropped then encoded. Did I miss something??

Yeah, 16x9 (or 1.78:1) for 704 is 396, not 256. This is 2.75:1 which is wider than 2.35:1 ratio which is considered Extra wide film.
